What information does CSE need?

When a custodial parent applies for child support services, the caseworker will need key pieces of information to open or assist in a child support case. The most important pieces of information needed about the non-custodial parent are:

  • Social Security Number
  • Full name
  • Date of birth
  • Current or last known address and phone number
  • Current or last known employer and address

Additional information that will be helpful includes:

  • Marriage license
  • Acknowledgment of paternity (or order of filiation by a court)
  • Divorce decree or separation agreement
  • Copies of any existing child support orders
  • Income information (tax records, paycheck stubs, etc.)
  • Child-related expenses and needs
  • Child(ren)'s information such as birth certificate, Social Security Number, health insurance coverage
